Ongoing Support Across a person’s Lifespan through Committed and Caring Family Support, Case Management and Specialized Services
For people ages 3 and above, your first experience with Community Bridges after Intake and Eligibility is through Individual and Family Support Services. We support people throughout Merrimack County of any age and eligible disability, whether developmental disability or acquired brain disorder. We offer a wide array of supports including the development of positive school district collaboration, researching and selecting providers of needed services as well as seeking community resources and opportunities, which you and your family can choose from and combine creatively to meet your priorities as you transition through each stage of life.
We believe that a full range of life experiences and opportunities are available to you, and we are committed to supporting you to align the services you need to live the full life you envision. Our Case Managers are trained to use Person-Centered Thinking strategies, and to facilitate Person-Centered Planning. Person-Centered processes enable people with disabilities to identify the personal life outcomes they want to achieve, to gain experience over their lifespan, and to work with their personal support network to choose pathways to success. A Person-Centered approach is critical to promoting and enhancing your personal choice, decision-making and direction.
The Individual and Family Support Department is made up of the following Case Management teams:
