Medical Support Homes

For those who need more health and physical support, Community Choices’ Residential and Day Services (RDS) offers comfortable, accessible homes. We offer opportunities and assistance so they can participate in work, volunteer, or social events. As with all our person-centered programs, the individual’s needs are paramount and personalized.

Each home has accessible vans, lifts and ramps, and is located within our service area, encouraging family involvement in their loved one’s care. We offer 24-hour monitoring for health conditions and provide an on-call system to assist in emergencies. We develop partnerships with family physicians or other doctors to support individuals in our Residential and Day Services Program. Medication administration is handled through a community-based approach aided by the care of an Agency Nurse Trainer.
