Welcome to Community Bridges.  A new diagnosis or learning about a new system of care can be a scary time, but it doesn’t have to be. At Community Bridges we support individuals with developmental disabilities and acquired brain disorders, from birth through the lifespan.    We are here to assist you through every stage.  Whether you are joining us through our Early Supports and Services, our Family Support Program, or as an adult, we want you to feel secure in our being there to support you.

You may be looking for assistance with case management, special education, residential or day supports, or even just a listening ear-we are here for you.  Our staff is dedicated to making sure that you get the proper resources and referrals that you need to help make the transition a smooth one.  We are committed to making sure you are a part of your local community in every way.
